This concept always generates controversy. In my opinion the intramucosal stage of colorectal cancer needs to be recognised and brought in line with upper gastrointestinal cancers so that we can properly compare and risk stratify all early GI cancers. Histopathologists are critical in this diagnosis and it should not be over-diagnosed.

Agree Tim. Intramucosal carcinoma WITHOUT invasion is a nonsensical concept. I called something lamina propria invasive and the surgeon kept on saying “so it’s benign then?” I really do feel lamina propria-invasive adenocarcinoma is an adequate term that conveys what the lesion is (not high-grade dysplasia or carcinoma in situ but a step beyond) and the extent of penetration.
